Please use this form to request a book or AV item not available in the Old Colony Library Network (OCLN).

• You must be a registered Paul Pratt Memorial Library (PPML) cardholder to use this form.
• Please ensure that the item(s) you are requesting are not available locally.
• First check the Online OCLN catalog (http://www.ocln.org/wp-public/) to find the item you want. You will need your library card number and PIN.
• If not available in the OCLN network, please check the Virtual Catalog (http://www.ocln.org/wp-public/?page_id=39). You will need your library card number.
• If the item is not in either online catalog, please use this online form or speak directly with library staff.
• When the item(s) are available, you will be notified by telephone or email. All requested items must be picked up at the PPML Circulation Desk.
• Please submit a separate request for each item you would like us to borrow.
• If you need a magazine or newspaper article, please use the ILL Article Request form.
